A PhD fellowship is available at Steno Diabetes Center Copenhagen (SDCC), Gentofte Hospital, University of Copenhagen, Denmark. The position is open for medical doctors with interest in clinical research within the cross field of monogenetic diabetes and modern treatment by September 1st, 2021.

The project is a close collaboration between several research environments within the Capital Region of Denmark, namely SDCC, Center for Clinical Metabolic Research CCMR) and the Novo Nordisk Foundation Center for Basic Metabolic Research, University of Copenhagen, Denmark. The clinical studies will be carried out at SDCC/CCMR where researcher and technical staff have a pronounced expertise and devotion to clinical metabolic research. The combination of a high-quality research environment together with a friendly and sociable atmosphere is characteristic for both SDCC and CCMR.

The project -will include patients with mutations in hepatocyte nuclear factor-1 alpha (HNF1A) which accounts for 0.5% of all cases of diabetes. The risk of diabetic complications in patients with HNF1A-MODY are comparable to that of patients with type 2 diabetes, and glycaemic control and management of risk factors are crucial for a good prognosis. First-line treatment of HNF1A-MODY is sulfonylurea (SU); however, there is limited evidence on second-line and third-line treatment options. Furthermore, studies have demonstrated that glycaemic control of a large proportion of patients with HNF1A-MODY are not at target and that ~35% of patients are treated with insulin although SU has been shown to be superior in terms of glycaemic control. Thus, there is a great potential for optimising glycaemic control by applying modern glucose-lowering agents which may reduce the risk of both microvascular and macrovascular complications in patients with HNF1A-MODY. Sodium glucose linked transporter 2 inhibitors (SGLT2i) and glucagon-like peptide 1 receptor agonists (GLP-1RA) are known for their cardiovascular and renal protective effects in type 2 diabetes. In patients with HNF1A-MODY, SGLT2i long-term efficacy and safety is currently unknown. In this PhD-proposal, we aim at delineating the potential of SGLT2i in HNF1A-MODY and we will plan a multiple therapy study with treatment based on SU, incretin-based therapy and SGLT2i in patients with HNF1A-MODY in a large interventional setting.

The job - the PhD-project contains three individual clinical studies. The PhD student will work closely together with the researchers in the research group and be responsible for writing protocols, recruiting participants, collecting and analysing data and finally writing manuscripts aimed for publication in scientific journals.

Terms - the PhD candidate should be a medical doctor (MD) and will be offered a full-time position for a period of three years. Employment will be in accordance with the relevant collective agreement within the Capital Region of Denmark.
